Mammary carcinoma is a malignant tumour arising from mammary tissue. In cats, the great majority of mammary tumours are malignant, and many behave aggressively. They are most common in older female cats, especially those that were not desexed early in life. Siamese and some related cats have been reported at increased risk.
A tumour may begin as a small firm nodule associated with one mammary gland. Cats have two chains of glands extending from the chest to the groin, and more than one gland can be affected. As tumours grow they may become fixed, ulcerated or painful. Spread commonly occurs to regional lymph nodes and lungs.
Hormonal exposure influences risk. Desexing before the first heat markedly reduces the likelihood of developing mammary carcinoma, while protection decreases as desexing is delayed through later reproductive cycles. Desexing an older cat does not remove a tumour that has already developed, but reproductive history remains relevant to prevention.
Tumour size at diagnosis strongly affects prognosis. Small masses treated before spread generally have a better outcome than large ulcerated tumours. This makes early detection unusually important. A mammary lump should never be watched for months simply because it is small or not painful.
The feline disease is notably different from the mixed benign-and-malignant pattern seen in dogs. Because most feline mammary masses are malignant until proven otherwise, the threshold for investigation should be low. Waiting for a small nodule to become painful or ulcerated can allow a treatable local tumour to become metastatic.
Prevention is strongest when female cats are desexed early, ideally before repeated ovarian hormone exposure. Early detection offers the best chance for surgery before metastatic disease is established. Cats generally adapt well after healing, although extensive mammary surgery can temporarily produce swelling, wound tension and restricted movement.
Owners can check the mammary chain gently during routine grooming, particularly in middle-aged and older female cats. Any firm nodule, asymmetry, swelling or ulcer associated with a mammary gland should be examined promptly.
The veterinarian palpates every mammary gland and regional lymph nodes because multiple tumours can occur. Chest radiographs or CT are used to look for spread to the lungs before major surgery. Abdominal imaging may be recommended depending on the case.
Fine-needle aspiration can provide preliminary information, but histopathology is required for definitive tumour diagnosis and grading. The size of the primary mass is recorded because it has prognostic importance. Small tumours should not be allowed to grow while waiting for them to become easier to diagnose.
There is no blood screening test for mammary carcinoma. For cats already at risk, physical examination is the practical screening method.
Diagnosis begins with examination and tissue sampling. Because cytology may not reliably distinguish all mammary lesions, histopathology after biopsy or surgery is important. Staging commonly includes chest imaging and assessment of lymph nodes to look for metastasis.
Surgery is the main treatment when there is no evidence that disease is too advanced for meaningful local control. Because feline mammary carcinoma often spreads within the mammary chain, surgery may involve removal of an entire chain rather than simply removing the visible lump. Bilateral disease may require staged procedures.
Histopathology confirms tumour type, grade and margins. Regional lymph nodes may also be assessed. Chemotherapy is considered for many cats because of the high metastatic risk, particularly when tumours are large, high grade or have spread to lymphatic tissue. The specific protocol is discussed with a veterinary oncologist.
Pain relief and wound care are important after extensive mammary surgery.
Follow-up includes checking the remaining mammary tissue and repeating chest imaging when indicated. Prognosis is strongly linked to tumour size and spread at diagnosis, so early surgery offers the greatest benefit. Prevention through early desexing remains more effective than any treatment once carcinoma has developed.
If metastatic disease is already extensive, palliative treatment may be more appropriate than radical surgery, with decisions centred on pain, breathing, appetite and expected quality of life.
