Lymphoma is cancer of lymphocytes, a type of white blood cell. It is one of the most common cancers in cats and can arise in many organs. Gastrointestinal lymphoma is particularly common, but disease may also involve the chest, kidneys, nose, nervous system, skin or multiple lymph nodes.
Clinical signs depend on location. Intestinal lymphoma can cause weight loss, vomiting, diarrhoea and reduced appetite. Mediastinal lymphoma can produce breathing difficulty from a mass or pleural fluid. Kidney involvement may cause enlargement and renal dysfunction. Nasal disease can cause discharge or noisy breathing.
Not all feline lymphoma behaves the same way. Low-grade intestinal lymphoma may progress slowly and can resemble inflammatory bowel disease, whereas high-grade lymphoma grows more aggressively and needs more intensive treatment. Feline leukaemia virus was historically associated with many lymphoma cases, particularly in younger cats. Modern FeLV control has reduced virus-associated disease in many regions, but viral status still influences the pattern, age distribution and wider health context of feline lymphoma.
The outlook depends on tumour grade, site and response to treatment. Lymphoma should therefore not be treated as one uniform diagnosis. A cat with low-grade intestinal disease can have a very different treatment plan and prognosis from a cat with rapidly progressive mediastinal or renal lymphoma.
The disease can also arise without obvious enlargement of peripheral lymph nodes. This is particularly true for gastrointestinal and renal forms, so owners should not expect to feel a lump. Persistent functional changes—weight loss, vomiting, diarrhoea or appetite change—may be the first evidence that lymphoid cancer is developing internally.
Persistent weight loss, vomiting, diarrhoea, enlarged lymph nodes, breathing difficulty and organ enlargement can reflect lymphoma at different anatomical sites.
Routine blood tests may reveal anaemia or other abnormalities but cannot confirm lymphoma by themselves.
Ultrasound is particularly useful for gastrointestinal, renal and abdominal lymphoma, while chest radiographs or ultrasound assess mediastinal disease and pleural fluid. Fine-needle aspiration of accessible masses or lymph nodes can provide a rapid diagnosis. Intestinal thickening may require biopsy because inflammatory bowel disease and low-grade lymphoma can look very similar on imaging.
FeLV testing is appropriate when status is unknown or when lymphoma is diagnosed. Additional staging tests are chosen according to tumour location and may include bone marrow assessment, advanced imaging or specialised pathology.
There is no general screening test for healthy cats that detects lymphoma early. Monitoring weight and appetite is valuable because gradual loss can precede obvious illness. A cat with chronic gastrointestinal signs that repeatedly relapses despite appropriate therapy should be reassessed rather than assumed to have uncomplicated inflammation forever.
Diagnosis requires sampling affected tissue. Fine-needle aspiration can diagnose some forms, while intestinal disease may require endoscopic or surgical biopsy to distinguish lymphoma from chronic inflammation. Imaging helps identify the extent of disease.
Treatment depends on lymphoma type and location. High-grade lymphoma is commonly treated with multi-drug chemotherapy, which aims for remission rather than simply slowing visible tumour growth. Cats often tolerate chemotherapy better than people expect, because veterinary protocols prioritise quality of life and generally cause less severe nausea and hair loss.
Low-grade intestinal lymphoma is often managed with oral chlorambucil and corticosteroid therapy, sometimes for prolonged periods. Nutritional support and vitamin B12 supplementation may be important when intestinal absorption is impaired. Localised nasal lymphoma can respond well to radiation therapy.
Pleural effusion, severe anaemia, dehydration or inability to eat may need stabilisation before anti-cancer treatment begins. FeLV-positive status and concurrent disease influence treatment decisions but do not automatically rule out therapy.
Response is monitored with examination, body weight, blood tests and repeat imaging appropriate to the tumour site. Prognosis varies widely: some low-grade intestinal cases remain controlled for years, while aggressive high-grade lymphoma has shorter expected remission. Decisions should be based on tumour biology, treatment response and the cat’s day-to-day wellbeing.
Supportive care for nausea, appetite and hydration is often needed alongside chemotherapy and can make a major difference to whether the cat maintains normal daily behaviour during treatment.
