Feline injection-site sarcoma is an uncommon but aggressive tumour that develops in areas where injections or other local inflammation have occurred. Most are high-grade fibrosarcomas or related soft-tissue sarcomas. Vaccines have historically received the most attention, but sarcomas have also been reported after other injections, implants and chronic local irritation.
The cancer is rare compared with the number of injections given to cats, so necessary vaccination and medication should not be avoided. The clinical concern is a persistent mass that develops at an injection site and continues to grow.
Normal post-injection inflammation should contract as healing completes. A sarcoma instead continues recruiting abnormal connective-tissue cells and enlarges beyond the expected healing period. This infiltrative growth, rather than surface size alone, explains why apparently small masses can become difficult local cancers.
The tumour and surrounding inflammatory response can become fixed to deeper tissue as they enlarge. Repeated disturbance may add scar formation around boundaries that were already indistinct. Local recurrence then tends to involve a wider and less mobile field, while metastatic behaviour remains separate from the immediate problem of deep infiltration.
These sarcomas are locally aggressive and have a meaningful risk of recurrence. Metastasis can occur, particularly to the lungs.
The rarity of the tumour is important context. Vaccines prevent common and serious infectious disease, while injection-site sarcoma affects only a very small proportion of cats. Good practice therefore balances necessary preventive care with appropriate injection-site selection and monitoring rather than avoiding vaccination altogether.
A later mass may arise within tissue altered by an earlier injection even when the intervening skin appeared completely normal. An unplanned marginal removal can disturb an infiltrative first mass and make later local control more difficult.
Any post-injection lump should be monitored for size and duration. Most temporary inflammatory swellings resolve, but a mass that lasts three months, exceeds two centimetres or continues growing one month after an injection meets commonly used criteria for biopsy or further investigation.
Measurement should be recorded rather than estimated. Photographs and notes on injection location can help track change.
Fine-needle aspiration may be inconclusive because sarcomas do not always shed many cells. Incisional biopsy is often preferred for a suspicious mass, positioned so the biopsy tract can be removed during definitive surgery. Advanced imaging such as CT is useful before treatment because the tumour can extend farther into tissue than the visible lump suggests.
Chest imaging is used to assess for metastatic spread. There is no blood screening test for injection-site sarcoma.
These tumours send microscopic extensions into surrounding tissue, which makes complete removal difficult if surgery is not planned widely from the start. The centre may feel discrete even while microscopic tumour extensions spread much farther through fascia and muscle. Early recognition, appropriate biopsy and referral for planned wide surgery provide a better chance of local control than repeatedly removing recurrent small lumps.
Definitive treatment usually centres on aggressive surgery with wide margins, often combined with radiation therapy. Because microscopic tumour extensions can reach beyond the palpable mass, the first well-planned surgery offers the best chance of durable control. Referral to a surgical or oncology centre is often recommended before the mass is removed.
Radiation can be used before or after surgery depending on tumour location and margin status. Chemotherapy may be considered for high-risk disease, although its benefit varies. Pain control is provided throughout treatment.
When the tumour occurs on a limb or distal tail, radical surgery can sometimes achieve better margins than procedures on the trunk, but treatment must be individualised. Repeat marginal excisions after each recurrence generally make local control harder.
Vaccination remains important. Risk is minimised by using current vaccine guidelines, avoiding unnecessary injections and placing injections in sites that allow treatment if a tumour rarely develops. Owners should monitor injection sites without becoming fearful of routine veterinary care. Prognosis is best when a small first tumour is diagnosed early and treated with appropriately planned local therapy.
After treatment, the surgical site is monitored for any new firmness or regrowth because local recurrence can begin beneath apparently normal skin.
