Inflammatory bowel disease, or IBD, is chronic inflammation of the stomach or intestines that produces persistent gastrointestinal signs after other causes have been excluded. The inflammatory cells infiltrate the intestinal wall and interfere with digestion, absorption and normal motility. In cats, the term is used carefully because low-grade intestinal lymphoma can look very similar.
Clinical signs include chronic vomiting, diarrhoea, weight loss, reduced appetite or, in some cats, increased appetite despite weight loss. Signs may wax and wane. Cats with small-intestinal disease can develop low vitamin B12 because the ileum and pancreas are involved in normal cobalamin handling.
There is no single cause. Abnormal immune response to intestinal bacteria or food antigens is thought to contribute, influenced by genetics and the intestinal microbiome. Parasites, food-responsive disease, pancreatic disease and infection must be considered before idiopathic inflammatory disease is diagnosed.
IBD is generally a chronic manageable condition rather than a quick cure. The aim is to control inflammation, maintain body weight and identify the least intensive treatment that keeps the cat comfortable.
The overlap with lymphoma is clinically important because both can produce thickened intestines, low vitamin B12 and chronic weight loss. This is why a long-standing “sensitive stomach” diagnosis deserves reconsideration when weight continues to fall.
Persistent vomiting, diarrhoea and unexplained weight loss can occur even while a cat continues to eat. Owners can record vomiting frequency, stool quality and body weight because chronic gastrointestinal disease is often underestimated when individual episodes seem minor. Early detection means taking chronic patterns seriously and tracking weight. Persistent signs despite temporary anti-vomiting relief can reflect lymphoma, pancreatitis or another disorder that mimics inflammatory bowel disease. Many cats live comfortably for years, but relapses are common.
Initial testing commonly includes blood count, chemistry, thyroid testing in older cats, faecal parasite assessment and vitamin B12. Pancreatic testing may be added because pancreatitis and exocrine pancreatic insufficiency can overlap with intestinal disease.
Abdominal ultrasound evaluates bowel thickness, layering, lymph nodes, pancreas and liver. Fine-needle samples can help in selected cases, but biopsy is often required to distinguish IBD from low-grade lymphoma with confidence. A structured diet trial may be performed before invasive testing when food-responsive disease is plausible.
There is no screening test for healthy cats.
Blood tests, faecal testing and abdominal ultrasound are commonly used during investigation. Ultrasound can show thickened bowel layers or enlarged lymph nodes but cannot always distinguish IBD from lymphoma. Definitive classification often requires intestinal biopsy, either endoscopically or surgically, together with expert histopathology. A diagnosis should not be used as a catch-all label for repeated vomiting without an appropriate work-up, especially in an older cat losing weight. In some cases only biopsy and detailed pathology can separate them confidently.
Treatment often begins with a highly digestible, novel-protein or hydrolysed diet depending on the cat’s history. The diet trial must be fed consistently to judge whether food-responsive inflammation is contributing. Vitamin B12 is supplemented when low because deficiency can worsen appetite and intestinal function.
Cats with persistent inflammatory disease may require corticosteroid therapy, usually prednisolone, to suppress intestinal inflammation. Other immune-modulating drugs are considered when response is incomplete or steroid side effects become problematic. The treatment plan changes if biopsy identifies lymphoma instead.
Anti-nausea medication, appetite support and fluids are used during flares, but maintaining calorie intake and body weight is central. Antibiotics are not a routine long-term treatment for uncomplicated IBD and are reserved for specific indications.
Follow-up uses body weight, stool, vomiting frequency and blood tests rather than judging success from one good week. Medication is tapered carefully when control is achieved. A change in the previous pattern or loss of response should prompt reassessment for lymphoma, pancreatitis or another concurrent disorder.
Because long-term corticosteroids can affect glucose metabolism and immune function, cats receiving them chronically may need periodic blood and urine monitoring.
