Hypothyroidism means thyroid hormone activity is too low. Naturally occurring hypothyroidism is uncommon in adult cats. Most adult cases develop after treatment for hyperthyroidism, particularly after radioactive iodine, thyroid surgery or excessive antithyroid medication. Congenital hypothyroidism is a separate, rare disorder seen in kittens.
Adult cats may become lethargic, gain weight without an obvious increase in food intake, seek warm places and develop a dry or poorly groomed coat. Heart rate can be slower than expected. The signs are often subtle and can resemble ageing or chronic kidney disease.
Congenitally affected kittens have a different presentation. Thyroid hormone is essential for normal skeletal and nervous-system development, so kittens may show poor growth, delayed tooth eruption, short limbs, a broad head, constipation and delayed mental or motor development. Without early treatment, some developmental changes become permanent.
A low total T4 alone does not prove hypothyroidism because many non-thyroid illnesses can suppress circulating thyroid hormone. Measuring TSH and considering previous hyperthyroidism treatment, clinical signs and repeat results helps distinguish true deficiency.
The condition is treatable with thyroid hormone replacement. The need for treatment depends on whether the deficiency is clinically meaningful and persistent. In a cat with kidney disease, restoring an appropriate thyroid level must be balanced carefully with renal monitoring.
Post-treatment hypothyroidism is important because it can be clinically meaningful even when the original hyperthyroidism has been successfully cured. A cat that is technically no longer hyperthyroid may still need active thyroid management if hormone levels fall too low and the change contributes to lethargy, weight gain or worsening renal function.
Persistent low T4, rising TSH, weight gain, lethargy or worsening kidney values can indicate clinically important hypothyroidism rather than successful control of hyperthyroidism. Congenital hypothyroidism is rare, but early recognition is particularly important because thyroid hormone is needed for normal skeletal and neurological development.
Cats treated for hyperthyroidism should have thyroid levels and kidney function rechecked according to the treatment protocol.
A low total T4 should not be interpreted in isolation. Severe unrelated illness can lower thyroid hormone temporarily, so the veterinarian considers the cat’s history, clinical signs and other laboratory abnormalities. Measurement of feline TSH and repeat thyroid testing can help confirm whether genuine deficiency is present.
Kittens with poor growth, delayed development, constipation, unusual body proportions or delayed tooth eruption require prompt investigation.
There is no reason to screen every healthy adult cat routinely for naturally occurring hypothyroidism. Monitoring is targeted mainly to cats previously treated for hyperthyroidism and kittens with compatible developmental abnormalities. Once replacement therapy begins, T4 and clinical response are reassessed so the dose can be adjusted without creating excessive thyroid hormone levels.
Because hypothyroidism can reduce kidney blood flow, persistent low thyroid hormone after hyperthyroidism treatment may make kidney dysfunction more apparent or worsen laboratory kidney values. Diagnosis requires interpretation of thyroid hormone measurements in context.
Confirmed hypothyroidism is treated with synthetic thyroid hormone, usually levothyroxine. The dose is tailored to the individual cat and adjusted according to clinical response and follow-up thyroid testing. Cats that become hypothyroid after hyperthyroidism treatment may need temporary or permanent supplementation depending on whether thyroid function recovers.
Kidney function deserves particular attention. Correcting hypothyroidism can improve metabolic function but may alter renal blood flow and laboratory values, so creatinine, SDMA and other kidney measures are reassessed during dose adjustment. The aim is to restore an appropriate thyroid state rather than push hormone levels above normal.
Congenitally affected kittens should begin treatment as early as possible. Growth, activity, bowel function and skeletal development are monitored closely. Delayed treatment may mean some developmental abnormalities cannot be fully reversed, even when thyroid levels later normalise.
Medication is generally given long term when permanent deficiency is present. Owners should not change or stop the dose based only on weight or activity without testing, because both under-treatment and excessive replacement can cause problems. With appropriate therapy, adult cats usually improve in energy, coat quality and weight regulation. Kittens treated early have the best chance of more normal development.
