Hepatic lipidosis is a potentially life-threatening liver disorder that develops when large amounts of fat accumulate within liver cells. It most often occurs after an overweight cat stops eating or eats far too little for several days. The body mobilises stored fat for energy, but the feline liver cannot process the sudden load efficiently.
The condition may begin because of another illness, stress or environmental change that suppresses appetite. Pancreatitis, diabetes, gastrointestinal disease and many other disorders can trigger the initial food refusal. In some cats no single precipitating disease is identified.
Signs include prolonged poor appetite, rapid weight loss, lethargy, vomiting, dehydration and jaundice. The cat may lose muscle quickly while remaining visibly overweight elsewhere. Liver dysfunction can alter clotting and electrolyte balance, and severe cases become profoundly weak.
The central treatment is nutrition. This is a major difference from older approaches that rested the gastrointestinal system. Cats with hepatic lipidosis need a reliable calorie supply, often through a feeding tube, while the liver gradually clears accumulated fat. Recovery can take weeks.
The condition is highly preventable if food refusal is addressed early. An overweight cat that stops eating should not be watched for several days in the hope that appetite returns on its own.
The liver can recover remarkably well if the cat survives the period of severe fat accumulation and receives sustained nutrition. This makes the condition different from many irreversible liver diseases. The challenge is maintaining safe calorie intake long enough for metabolic processing to normalise while the original appetite-suppressing problem is treated.
Any cat that has eaten very little or nothing for more than a short period deserves attention, especially if overweight. Daily food intake is more informative than simply seeing the cat approach the bowl. Jaundice, vomiting or rapid weight loss increases urgency.
Blood tests assess bilirubin, liver enzymes, electrolytes, glucose and clotting risk. Ultrasound evaluates liver appearance and looks for pancreatitis, intestinal disease, bile-duct problems or other causes of anorexia. Fine-needle aspiration can support the diagnosis in appropriate cases.
Body weight and muscle condition should be recorded because rapid loss is a key clue. Potassium and phosphorus require monitoring when nutrition is restarted because severely depleted cats can develop refeeding complications.
There is no screening test for hepatic lipidosis before food intake falls. Prevention is early recognition of anorexia. Owners should contact a veterinarian when a cat stops eating rather than repeatedly changing foods at home for several days. Early nutritional intervention can prevent a reversible appetite problem from becoming serious liver disease.
Diagnosis uses blood tests showing characteristic liver-value abnormalities, bilirubin and other metabolic changes. Ultrasound assesses the liver and searches for the illness that caused the cat to stop eating. Fine-needle sampling or biopsy may be used when other liver disease must be distinguished.
Treatment centres on providing adequate calories and protein consistently. A feeding tube, commonly an oesophagostomy tube, is often the safest and most reliable way to meet nutritional needs for several weeks while the cat’s appetite and liver function recover. Tube feeding avoids repeated stressful force-feeding.
Calories are introduced gradually in debilitated cats while electrolytes such as potassium and phosphorus are monitored. Anti-nausea medication, appetite support, fluids and vitamin supplementation are used as needed. Any underlying trigger—pancreatitis, diabetes, intestinal disease, infection or stress—must be treated at the same time.
Force-feeding by syringe can create food aversion and aspiration risk and is generally not a substitute for a well-managed feeding tube when prolonged support is required. Owners can usually learn tube feeding at home once the cat is stable.
Recovery is often good when intensive nutritional support begins before severe complications develop, but improvement takes time. The tube remains in place until the cat is eating enough independently and maintaining weight. Prevention of recurrence depends on managing the original cause and responding promptly to any future period of reduced food intake.
