A gastrointestinal foreign body is any swallowed object that becomes lodged in the stomach or intestines or causes injury as it passes. Cats commonly ingest string, thread, ribbon, hair ties, foam, small toys and other household items. Linear foreign bodies such as string are particularly dangerous because one end can anchor under the tongue or in the stomach while the intestine gathers along the material.
As the bowel bunches, the taut string can cut through the intestinal wall and cause leakage into the abdomen. A non-linear object can also block the stomach or intestine and prevent normal movement of food and fluid. The result may be vomiting, poor appetite, abdominal pain, lethargy and dehydration.
Some cats continue to pass small amounts of stool despite a partial obstruction, so defecation does not rule out a foreign body. String protruding from the mouth or anus should never be pulled because traction can worsen intestinal injury.
Foreign bodies are important because delay can convert a straightforward removal into bowel perforation, peritonitis and major surgery. Cats that repeatedly chew fabric, string or other non-food items may have pica, and prevention requires both removing access and considering medical or behavioural contributors.
Linear foreign bodies are particularly deceptive because the object can remain partly visible while most injury is hidden inside the abdomen. The absence of a complete blockage does not make them safe. Ongoing traction can progressively damage several intestinal loops even when small amounts of food or stool still pass.
Known access to string, ribbon, needles or hair ties increases concern even before severe signs develop. Early detection matters because removing an object before bowel tissue is damaged usually results in a simpler procedure and faster recovery.
Repeated vomiting, sudden loss of appetite, abdominal pain or unusual lethargy should prompt assessment when a cat may have swallowed an object.
The mouth, including beneath the tongue, is examined carefully for anchored string. Owners should not pull any visible linear material. Abdominal palpation may reveal pain or thickened loops, but normal palpation does not exclude obstruction.
Radiographs are commonly performed first. Because many foreign materials are not directly visible on x-ray, the veterinarian also looks for secondary gas patterns. Ultrasound can show intestinal plication, trapped material, fluid accumulation and altered motility. Repeat imaging may be needed when early findings are uncertain.
There is no screening test for healthy cats. Prevention is the practical strategy: secure sewing supplies, ribbons, elastic bands and small toys and monitor cats with pica.
Diagnosis uses history, physical examination and imaging. Radiographs may show an object directly or reveal abnormal gas patterns. Ultrasound is particularly useful for soft material, intestinal bunching and assessment of bowel movement. Blood tests identify dehydration and secondary metabolic changes.
Some objects in the stomach can be removed by endoscopy before they enter the intestine, avoiding abdominal surgery. Once an object causes intestinal obstruction, surgery is commonly required. The surgeon removes the foreign material and assesses whether any bowel has lost blood supply or perforated. Damaged sections may need to be resected and reconnected.
Before and after removal, intravenous fluids correct dehydration and electrolytes. Pain relief and anti-nausea medication support recovery. Antibiotics are used when bacterial contamination or perforation is present rather than automatically for every uncomplicated object.
Nutrition is restarted according to intestinal condition and the procedure performed. Cats with peritonitis or extensive bowel surgery require more intensive monitoring than those with a simple gastric removal.
Prevention after recovery is important because some cats swallow objects repeatedly. String, dental floss, ribbon, hair ties and small foam items should be stored securely, and play with string toys should be supervised. If pica persists, the cat should be assessed for gastrointestinal disease, nutritional issues and behavioural drivers. Any future visible string should again be treated as a warning, not pulled.
After surgery, appetite, vomiting, wound comfort and stool passage are monitored closely because leakage or postoperative ileus can become serious complications.
