Feline upper respiratory disease describes infection and inflammation affecting the nose, throat, eyes and upper airways. It is a syndrome rather than one single infection. Feline herpesvirus and feline calicivirus are major causes, while Chlamydia felis, Bordetella bronchiseptica and other organisms can contribute, particularly in shelters, breeding colonies and multi-cat environments.
Common signs include sneezing, nasal discharge, conjunctivitis, eye discharge, fever, reduced appetite and lethargy. Mouth ulcers are more suggestive of calicivirus, while marked conjunctivitis can occur with herpesvirus or Chlamydia. Young kittens and cats with reduced immune function may become more severely affected.
Loss of smell from nasal congestion can make a cat stop eating. This is clinically important because cats are vulnerable to complications when calorie intake falls. Thick discharge can also make breathing uncomfortable, and dehydration can develop when fever and poor appetite persist. Lower respiratory involvement or pneumonia is less common but more serious.
Many uncomplicated infections are self-limiting, but herpesvirus can remain latent for life and reactivate during stress. Vaccination does not prevent every infection, but it reduces the severity of major viral respiratory diseases. Good hygiene, appropriate isolation of acutely infectious cats and reducing crowding are important in group settings.
The syndrome is especially important in kittens because congestion, fever and oral or eye pain can rapidly reduce food intake. In an adult cat a mild episode may be inconvenient; in a small kitten the same degree of nasal blockage can contribute to dehydration, poor growth and a need for assisted nutrition.
Early detection relies on recognising the first respiratory and eye changes, especially in kittens or multi-cat environments. Corneal staining is important when herpesvirus-associated ulceration is possible. Young kittens can deteriorate more quickly than healthy adults.
Sneezing, watery or thick eye discharge, nasal discharge, reduced appetite, fever or oral ulcers should prompt assessment when signs are persistent, severe or affecting food intake.
The veterinarian examines the eyes, mouth, hydration and breathing and listens for evidence that disease has extended into the lower respiratory tract. Cats with significant lethargy, weight loss or prolonged illness may need blood tests or imaging.
PCR panels can be useful in outbreaks, catteries or unusual cases, but they are not necessary for every uncomplicated upper respiratory infection. A positive PCR does not always prove that the detected organism is the sole cause because some agents can be shed intermittently by carriers.
Vaccination status should be reviewed as part of prevention. New cats entering group housing may require health assessment and an appropriate quarantine period. Any cat with open-mouth breathing, pronounced respiratory effort, dehydration or inability to eat needs prompt veterinary care.
Diagnosis is often based on clinical pattern and exposure history. PCR testing can identify respiratory organisms in selected cases, but positive results need careful interpretation because some viruses can be carried or shed by cats without explaining every current sign.
Treatment depends on severity and the likely organism. Mild viral cases often need supportive care: keeping the nose and eyes clean, maintaining hydration, encouraging food intake and reducing stress. Warming aromatic wet food can help a congested cat smell and accept food. Cats that stop eating may need appetite support or assisted nutrition.
Eye disease is treated according to the lesion. Herpesvirus-associated conjunctivitis or corneal disease may require antiviral medication, while Chlamydia felis is treated with an appropriate antibiotic. Antibiotics are not automatically required for every viral respiratory infection, although secondary bacterial infection can justify them.
Nebulisation or humidification may help loosen nasal secretions in selected cats. Severe dehydration, pneumonia or inability to eat can require hospitalisation with fluids, oxygen and nutritional support.
In multi-cat homes, acute cases should have separate bowls, litter trays and bedding where practical, with careful hand hygiene to reduce spread. Stress reduction is particularly useful for herpesvirus carriers because stress can trigger recurrence. Vaccination remains important for reducing severity of core viral respiratory disease even though it does not guarantee complete prevention of infection.
Persistent anorexia should be treated actively rather than accepted as part of a cold, because cats that eat poorly for several days can develop serious secondary metabolic problems.
