Feline lower urinary tract disease, usually abbreviated FLUTD, is an umbrella term for disorders affecting the bladder and urethra rather than one single diagnosis. Cats with different underlying problems can show almost identical signs, including frequent trips to the litter tray, straining, passing small amounts of urine, blood in the urine, urinating outside the tray or licking the genital area.
Common causes include feline idiopathic cystitis, urinary stones, urethral plugs and, less commonly in younger cats, bacterial infection. Tumours and anatomical problems are additional possibilities, particularly in older cats.
Male cats deserve particular attention because their narrow urethra can become blocked by plugs, crystals, stones or inflammatory material. A urethral obstruction is an emergency. A blocked cat may make repeated unproductive attempts to urinate, vocalise, hide, vomit or become weak. Without relief, dangerous electrolyte changes, kidney injury and death can occur.
Stress, low water intake, indoor lifestyle, obesity and multi-cat conflict can contribute to recurrent lower urinary problems, especially idiopathic cystitis. The condition is therefore influenced by both physical and environmental factors.
FLUTD is best understood as a clinical syndrome requiring a cause-specific plan. Long-term management often combines hydration, diet, litter-tray optimisation, weight control and reduction of environmental stress.
Two cats with identical straining can therefore need completely different treatment. One may have stress-associated sterile bladder inflammation, another may have a dissolvable stone, and a third may be obstructed.
Early recognition depends on noticing changes in litter-tray behaviour. Repeated tray visits, straining, drops of urine, blood, crying during urination, genital licking and sudden urination on unusual surfaces are signs of lower urinary pain rather than simple misbehaviour. The most important distinction is whether urine is actually being passed. A male cat repeatedly straining without producing urine must be treated as an emergency.
Owners should not wait overnight to see whether the problem settles, because complete obstruction can deteriorate rapidly.
Veterinary investigation commonly includes palpation of the bladder and urinalysis. A fresh urine sample can reveal blood, crystals, urine concentration and inflammation. Urine culture is important when infection is suspected, particularly in older cats or those with concurrent disease. Imaging with radiographs or ultrasound is used when stones, masses or structural problems are possible.
For cats with recurrent episodes, body weight, water intake, diet, litter-tray access and household stress should be reviewed. Keeping a record of episode frequency and urine changes can help identify patterns. There is no single screening test for FLUTD in a symptom-free cat; early detection depends on close observation and investigation of each episode.
Because the outward signs overlap, it is unsafe to assume that every episode is “cystitis” without assessment. Diagnosis starts with history, examination and urinalysis. Depending on the cat, urine culture, blood tests, radiographs or ultrasound may be needed to identify infection, stones or another cause. That is why repeated urinary signs deserve a fresh clinical assessment rather than automatic reuse of a previous diagnosis or medication.
Treatment depends on the cause. A urethral obstruction requires emergency stabilisation, pain relief and restoration of urine flow, usually with catheterisation under sedation or anaesthesia. Fluid therapy and monitoring of kidney values and electrolytes are often needed until the cat is stable.
Cats with idiopathic cystitis are managed very differently from cats with stones or bacterial infection. Pain relief, increased water intake and environmental modification are central for idiopathic disease. Wet food, water fountains and multiple water stations can help increase moisture intake. Reducing conflict, providing predictable routines, adequate hiding places and enough litter trays may reduce stress-related recurrence.
Urinary stones may require a prescription diet, removal or another procedure depending on their type and location. Antibiotics are used when bacterial infection is demonstrated or strongly supported, not simply because blood is present in the urine.
Long-term management is individual. Weight control, appropriate diet, generous water access and clean, well-positioned litter trays are important for many cats. Recurrent episodes should be reassessed rather than automatically treated the same way each time, because the underlying cause can change. Any future episode where a cat cannot pass urine remains an emergency regardless of previous diagnoses.
