Feline idiopathic cystitis, or FIC, is inflammation and pain affecting the bladder where no bacterial infection, stone or other specific structural cause can be identified. It is one of the most common causes of lower urinary tract signs in young and middle-aged cats. The term “idiopathic” means that there is no single identifiable cause, not that the disease is imaginary or unimportant.
Affected cats may strain, urinate frequently, pass small amounts, have blood in the urine, lick the genital area or urinate outside the litter tray. Episodes can appear suddenly and may settle within several days, but recurrence is common. Male cats with FIC can also develop urethral obstruction if inflammatory material and urethral spasm prevent urine flow.
Stress has a strong association with FIC. Cats can be affected by conflict with other cats, changes in routine, limited access to preferred resources, lack of hiding places, boredom or unpredictable environments. The disease is thought to involve abnormal interactions between the nervous system, stress response and bladder lining rather than a simple infection.
Antibiotics are not useful for uncomplicated FIC because bacteria are not the cause.
Many cats improve substantially when their environment gives them more control, predictable access to food and water, sufficient litter trays, opportunities to hide and climb, and regular play.
That negative investigation is still clinically useful: it supports the diagnosis and helps avoid unnecessary antibiotics while directing attention towards pain relief, hydration and environmental management.
A male cat that cannot pass urine needs emergency care because obstruction cannot be distinguished safely at home from a painful but non-obstructive episode. It helps assess urine concentration, blood, crystals and evidence of inflammation. Keeping a diary of episodes can help identify triggers.
FIC is recognised through urinary signs, but diagnosis requires ruling out problems that look similar. Any cat straining, passing blood or repeatedly visiting the litter tray should be examined.
Urinalysis is usually the first diagnostic test. Urine culture may be recommended, particularly in older cats or those with previous urinary procedures or concurrent disease. Radiographs or ultrasound are used when stones, anatomical changes or masses need to be excluded.
For recurrent FIC, the household environment is part of the assessment. The number and location of litter trays, feeding areas, water sources, resting places and hiding opportunities should be reviewed, along with recent changes or conflict between cats.
There is no routine laboratory screening test for healthy cats that predicts FIC. Early detection means recognising subtle changes in litter-tray behaviour promptly and confirming that urine is still being produced. Frequent small urinations or house-soiling should never automatically be labelled as behavioural without medical assessment.
Diagnosis is made by excluding other causes of similar signs. Urinalysis is essential, and imaging or urine culture may be needed depending on age, history and recurrence. FIC is often frustrating because tests may show blood and inflammation without revealing a stone, infection or tumour.
An acute episode is treated primarily with effective pain relief and measures that support normal urination. Increasing moisture intake is helpful, often by feeding wet food and offering several appealing water sources. Some cats benefit from short-term medication selected by the veterinarian to reduce discomfort or urethral spasm.
Environmental management is a major part of long-term care. Litter trays should be large, clean, easy to access and distributed so one cat cannot block another. Food, water, scratching areas, beds, hiding places and elevated resting sites should be available in more than one location in multi-cat homes. Regular interactive play and predictable daily routines can reduce boredom and stress.
A urinary diet may be recommended when urine characteristics or crystal risk justify it, but diet alone does not address all aspects of FIC. Antibiotics are not indicated unless a bacterial infection has actually been identified.
Many episodes resolve, but recurrence is common. The goal is to reduce both frequency and severity by improving hydration and environmental security. Cats that develop repeated obstruction may require additional intervention, and some male cats ultimately need perineal urethrostomy when obstruction cannot be controlled by other means. Every episode of unsuccessful straining remains an emergency.
Management therefore focuses on pain control during episodes, increased water intake and reducing the factors that trigger stress.
