Feline herpesvirus type 1 is a highly contagious virus that primarily affects the eyes, nose and upper respiratory tract. It is a major cause of feline viral rhinotracheitis. After the initial infection, the virus becomes latent within nervous tissue and usually remains in the cat for life. Stress or illness can trigger renewed shedding and recurrent clinical signs.
Kittens often develop sneezing, fever, nasal discharge and marked conjunctivitis. Corneal ulceration is an important complication because herpesvirus can damage the surface of the eye. Recurrent disease may present mainly as one-sided or intermittent eye inflammation rather than a full respiratory illness. Chronic nasal damage after severe infection can contribute to persistent rhinitis in some cats.
Transmission occurs mainly through close contact with infected eye, nose and oral secretions. The virus does not survive indefinitely in the environment, but shared bowls, hands and contaminated surfaces can assist spread during active shedding. Cats may shed virus without obvious signs during reactivation.
Vaccination is a core preventive measure. It does not completely prevent infection or latency, but it reduces disease severity. The long-term challenge is therefore not eradication of the virus from an infected cat, but managing recurrences and protecting the eyes, hydration and appetite during active episodes.
Latent infection explains why a cat can develop recurrent signs years after the original kitten illness without a new exposure. Reactivation can follow boarding, moving house, surgery, illness or social conflict. That pattern is characteristic of herpesvirus biology and helps explain why management often includes stress reduction as well as medication.
Characteristic branching or superficial lesions can support suspicion of herpesvirus, although not every ulcer has a classic appearance. In shelters or breeding environments, separating actively symptomatic cats, reducing crowding and maintaining hygiene can limit transmission. Early detection relies on noticing recurrent eye or nasal signs quickly.
Kittens and newly acquired cats should be monitored for sneezing, eye discharge, swollen conjunctiva and reduced appetite, particularly after transport, rehoming or other stress. Corneal pain, squinting or a cloudy eye warrants prompt examination because herpesvirus can cause ulcers that need specific treatment.
A veterinarian may perform fluorescein staining to identify corneal ulceration. PCR testing may be helpful in severe, recurrent or group outbreaks, but a positive result is not always proof that herpesvirus explains all current signs.
Vaccination should follow current feline guidelines and be maintained according to individual risk.
There is no screening test that predicts when a latently infected cat will relapse. Any cat that stops eating, becomes dehydrated or develops marked breathing difficulty requires more urgent veterinary support than a mild self-limiting recurrence.
Diagnosis is often clinical, particularly when corneal lesions and respiratory signs fit the pattern. PCR can detect viral DNA, but positive results must be interpreted with the history because latent or intermittently shedding cats may test positive even when another problem contributes to current signs.
Mild episodes may need supportive care only, including cleaning eye and nasal discharge, maintaining hydration and encouraging food. Cats with painful eye disease require veterinary examination because corneal ulcers change treatment choices and some common eye medications can worsen herpetic disease.
Antiviral therapy is used when ocular or respiratory disease is clinically significant. Famciclovir is commonly used systemically, and topical antiviral medication may be selected for specific eye disease. Antibiotics do not treat the virus but may be prescribed when secondary bacterial infection is present.
Stress reduction is an important part of long-term management. Predictable routines, adequate hiding spaces and careful introductions can reduce triggers in susceptible cats. Recurrences often become less severe with age, although some cats have chronic ocular or nasal problems.
In group environments, symptomatic cats should be managed to reduce contact with susceptible animals, particularly young kittens. Food bowls, bedding and hands should be cleaned appropriately. Vaccination of healthy cats reduces disease severity but does not eliminate latent infection. The goal in an infected cat is rapid control of painful eye disease, preservation of appetite and hydration, and reduction of recurrence frequency rather than complete viral eradication.
