Feline asthma is a chronic inflammatory disease of the lower airways in which the bronchi become hypersensitive, narrowed and prone to spasm. Inflammation thickens the airway lining and increases mucus production, while contraction of bronchial muscle further restricts airflow. The result can range from intermittent coughing to severe breathing difficulty.
Cats may cough in a low crouched position, wheeze, breathe faster than normal or show increased abdominal effort. Owners sometimes mistake coughing for an attempt to vomit a hairball because the posture can look similar. During a severe attack, a cat may breathe with the mouth open, extend the neck or become cyanotic. Open-mouth breathing in a cat is an emergency.
The trigger is not always identified. Smoke, aerosols, fragrances, dusty litter and other airborne irritants can worsen signs in susceptible cats, but eliminating one product does not necessarily cure the disease. Asthma must also be distinguished from chronic bronchitis, lung parasites, infection, heart disease and other causes of cough or breathing difficulty.
Airway sampling may be recommended to identify eosinophilic inflammation and exclude infection. A cat in respiratory distress is stabilised before stressful diagnostic procedures.
Asthma is generally a manageable chronic disease rather than a condition that is permanently cured. Good control reduces airway inflammation and the frequency of attacks. Severe untreated bronchospasm, however, can become life threatening very quickly.
Asthma can also wax and wane. A cat may be normal between attacks, then cough repeatedly after exposure to an irritant or without an obvious trigger. This intermittent pattern does not make the disease trivial; chronic airway inflammation can remain present even when outward breathing appears completely normal.
Heart disease is considered when breathing signs overlap with cardiac conditions. Early detection depends on recognising repeated cough or subtle breathing changes.
Recurrent coughing, wheezing or increased breathing effort should be investigated rather than assumed to be hairballs. Video recorded at home can be valuable because cats may not cough during a veterinary visit. A resting or sleeping respiratory rate that rises persistently above the cat’s normal baseline also deserves attention.
Chest radiographs are commonly used when asthma is suspected. They may show a bronchial pattern or hyperinflated lungs, but radiographs can occasionally appear relatively normal between episodes. Blood tests help assess general health, and parasite testing may be appropriate depending on exposure.
Airway sampling by bronchoalveolar lavage or another technique can help distinguish eosinophilic asthma from infection or other inflammatory disease. The procedure is undertaken only when the cat is stable enough for anaesthesia.
There is no useful screening test for healthy symptom-free cats. Open-mouth breathing, blue gums, collapse or severe effort requires emergency care and should not be delayed for home monitoring or an elective appointment.
Diagnosis commonly uses chest radiographs together with history and examination. Parasite testing and other investigations are selected according to exposure and location.
Long-term treatment focuses on suppressing airway inflammation. Corticosteroids are the main anti-inflammatory therapy and may be given by mouth initially or delivered by inhaler for ongoing control. Inhaled medication can reduce whole-body exposure, but the cat must be gradually trained to accept a spacer and mask.
Bronchodilators are used to relieve bronchospasm and are especially important during acute episodes, but they do not replace anti-inflammatory treatment. A cat in severe distress may need oxygen, injectable medication and minimal handling until breathing stabilises.
Environmental irritants should be reduced where practical. Smoke, vaping aerosols, strong fragrances, sprays and very dusty litter can worsen airway reactivity. Weight control is also useful because obesity can make respiratory effort more difficult.
Medication is adjusted to the lowest regimen that maintains good control rather than stopped abruptly whenever the cat appears better. Owners can monitor cough frequency and resting respiratory rate to identify deterioration early. Some cats have only occasional episodes, while others require lifelong daily therapy. With consistent treatment, many maintain a normal activity level and good quality of life.
A written action plan is useful for cats with previous severe attacks so the household knows which prescribed rescue medication can be given and when emergency transport is required.
