Exocrine pancreatic insufficiency, or EPI, occurs when the pancreas does not produce enough digestive enzymes to break food down normally. Nutrients then pass through the intestine without adequate digestion and absorption. In cats, chronic pancreatitis is an important underlying cause, although pancreatic tissue can be lost for other reasons.
Affected cats commonly lose weight despite a normal or increased appetite. Stools may become loose, bulky or unusually frequent, and the coat can become poor because of malnutrition. Some cats have vomiting or reduced appetite instead of the classic ravenous presentation, which can delay diagnosis.
Vitamin B12 deficiency is especially common in feline EPI because the pancreas contributes a protein needed for normal cobalamin absorption. Cats may fail to improve fully if enzyme replacement is started but vitamin B12 deficiency is not corrected. Folate and other nutritional abnormalities can also occur.
A low fasting fTLI supports pancreatic exocrine failure. Vitamin B12 is measured at the same time.
EPI is highly treatable even though the lost pancreatic tissue generally does not regenerate. Digestive enzymes are supplied with every meal, allowing nutrients to be absorbed again. Once treatment is established, many cats regain weight and normal stool quality. Persistent signs despite therapy should prompt review of dose, administration, cobalamin status and concurrent intestinal disease.
Because feline EPI is relatively uncommon, the diagnosis can be delayed when weight loss is attributed to intestinal disease alone.
A history of chronic pancreatitis increases suspicion but is not required. Markedly low concentrations are strongly supportive of exocrine pancreatic insufficiency. Routine chemistry can identify dehydration or concurrent liver and kidney abnormalities. Failure to gain weight despite a good appetite is a particularly useful clue. Most cats respond very well and can regain lost muscle and coat quality.
EPI should be considered in cats with unexplained weight loss, chronic diarrhoea or poor body condition, especially when appetite is normal or increased.
Fasting serum fTLI is the main diagnostic test. Vitamin B12 should be measured at the same time because deficiency is common and clinically important.
Abdominal ultrasound may show pancreatic changes and can identify inflammatory bowel disease, cholangitis or other conditions that commonly coexist in cats. Faecal parasite testing is appropriate when diarrhoea is present so treatable causes are not overlooked.
There is no routine screening test for healthy cats. Early diagnosis depends on tracking body weight and investigating chronic weight loss rather than assuming a thin older cat is simply ageing.
Serial body-weight measurements provide a simple way to judge whether digestion is improving once treatment starts and can reveal incomplete response before severe wasting returns.
The diagnostic test is feline trypsin-like immunoreactivity, usually abbreviated fTLI. Ultrasound can assess the pancreas and look for concurrent intestinal or biliary disease but cannot replace fTLI. The combination of poor body condition, altered stool and low cobalamin should prompt specific pancreatic testing. A simple fasting blood test can identify a very treatable cause of chronic malabsorption.
Treatment uses pancreatic enzyme replacement with every meal. Powdered enzyme preparations are commonly mixed thoroughly with food so digestion can begin normally. The dose is adjusted to stool quality, weight gain and clinical response rather than stopped once the cat initially improves.
Vitamin B12 deficiency is corrected with injectable or oral supplementation according to severity and follow-up results. Some cats require ongoing cobalamin support. Diet should be complete and palatable; severe restriction is usually unnecessary unless another gastrointestinal disease requires it.
If diarrhoea or weight loss persists despite adequate enzymes and B12, the veterinarian checks administration technique, dose, intestinal disease and other causes. Antibiotics are not routine treatment for EPI itself but may be used for a specific concurrent indication.
Enzyme replacement is usually lifelong because pancreatic exocrine tissue does not recover sufficiently. Owners should monitor weight and stool and ensure every meal receives enzymes. Long-term prognosis is favourable when treatment is given consistently and concurrent disease is controlled.
Enzyme powder can irritate the mouth in some cats if not mixed thoroughly, so administration technique should be reviewed if oral soreness develops.
