Epilepsy describes a tendency to have recurrent unprovoked seizures. A seizure results from sudden abnormal electrical activity in the brain and can cause loss of consciousness, stiffening, paddling, salivation, urination or brief focal signs such as facial twitching and abnormal behaviour. Not every seizure means a cat has epilepsy.
In cats, underlying structural brain disease is an important consideration. Tumours, inflammation, trauma, congenital abnormalities and vascular disease can all cause recurrent seizures. Metabolic problems such as low blood glucose or severe liver disease can also provoke seizures without primary brain epilepsy.
The period after a seizure can include disorientation, temporary blindness, hunger, restlessness or profound sleepiness. Most individual seizures stop within one to two minutes. A seizure lasting several minutes, repeated seizures without full recovery or clusters over a short period are emergencies because sustained seizure activity can cause overheating and brain injury.
The long-term plan depends on seizure frequency, severity and cause. Some cats need no daily medication after a single isolated seizure, while others require lifelong anticonvulsant treatment. The goal is usually to reduce frequency and severity to an acceptable level rather than guarantee that no seizure will ever occur.
Focal seizures deserve particular attention in cats because they may be brief and easily mistaken for odd behaviour. Repeated facial twitching, sudden chewing movements, staring episodes or episodic aggression can represent seizure activity even without full-body paddling. Video and pattern recognition are often the first steps towards diagnosis.
Video is extremely useful because fainting, sleep disorders and movement abnormalities can mimic seizures. Early detection means recognising clusters and prolonged seizures as emergencies. A seizure lasting about five minutes and repeated seizures without full recovery represent escalating neurological emergencies.
Owners should record the date, duration and appearance of every suspected seizure. The veterinarian also needs to know about toxins, medications, trauma and any behavioural or neurological change between episodes.
Blood testing assesses glucose, liver function, electrolytes and other metabolic causes. Blood pressure and infectious-disease testing may be appropriate according to age and history. A neurological examination that remains abnormal between seizures increases concern for structural brain disease.
MRI and cerebrospinal-fluid analysis are commonly recommended when seizures begin at an unusual age, are accompanied by neurological deficits or cannot be explained by routine testing. These investigations distinguish idiopathic epilepsy from tumour, inflammation and other intracranial disease.
There is no screening test for idiopathic epilepsy in a healthy cat before events occur.
Idiopathic epilepsy, where no structural cause is identified, does occur but is a diagnosis made after appropriate investigation. Diagnosis begins with a detailed history and ideally video of the event. Blood tests exclude metabolic causes. Neurological examination between seizures helps decide whether MRI and cerebrospinal-fluid analysis are needed to look for structural disease.
Treatment is based on cause and seizure pattern. Structural or metabolic disease is treated directly whenever possible. Daily anticonvulsant medication is considered when seizures are frequent, occur in clusters, are prolonged or significantly impair recovery and quality of life.
Phenobarbital is commonly used in cats, and levetiracetam is another important option. Drug choice depends on the cat’s health, seizure type and dosing practicality. Blood monitoring may be needed to check drug concentration and organ function, particularly with long-term phenobarbital.
Owners should give medication consistently because abrupt withdrawal can trigger seizures. A home rescue plan may include medication prescribed specifically for prolonged or cluster events. During a seizure, the cat should be protected from falls and nearby hazards but not restrained or have objects placed in its mouth.
Seizure diaries help judge whether treatment is working. Complete elimination is not always achievable; a meaningful reduction in frequency, duration and recovery time can still represent successful control. Any sudden increase in seizure activity, marked sedation or other suspected drug reaction requires reassessment and possible adjustment.
Routine follow-up also reviews whether the cat’s behaviour, appetite or coordination has changed between seizures, which can indicate progression of an underlying structural disease.
