Eosinophilic granuloma complex is a group of inflammatory skin and oral lesions associated with eosinophils, a type of white blood cell involved in allergic and parasitic responses. It includes eosinophilic plaques, eosinophilic granulomas and indolent ulcers. These patterns can look very different even though they share related inflammatory mechanisms.
Eosinophilic plaques are usually raised, red, moist and intensely itchy, often on the abdomen or inner thighs. Granulomas can form firm linear lesions on the back of the thighs, swollen areas on the chin or lips, or lesions within the mouth. Indolent ulcers classically affect the upper lip and may look dramatic while causing surprisingly little discomfort.
The lesion itself is not the final diagnosis. Flea allergy, food allergy, environmental allergy, mosquito hypersensitivity and other triggers can drive eosinophilic inflammation. In some cats no single trigger is found. Secondary infection may worsen lesions that the cat repeatedly licks or scratches.
This is particularly important for oral or ulcerative lesions in older cats.
A lesion can disappear after anti-inflammatory medication and then recur because fleas, dietary allergy or another driver remains. Long-term success therefore depends on identifying controllable causes rather than repeatedly treating each new lesion as an isolated skin problem.
The three classic lesion names describe appearance, not three completely separate diseases. A single allergic cat can develop different eosinophilic patterns at different times. Recognising that connection helps prevent each new plaque or lip lesion from being treated as an unrelated problem while the underlying trigger remains active.
Flea combing and strict parasite control are important because flea allergy is a common trigger even when adult fleas are not seen. Environmental atopic disease is considered after parasites and food have been addressed.
Any persistent lip ulcer, raised moist plaque or unusual linear skin lesion should be examined. Owners should note whether the cat is itchy, whether lesions recur seasonally and whether flea control or diet changes alter the pattern.
Cytology is a useful first test and can show eosinophilic inflammation, secondary bacteria or yeast. Fungal testing or mite investigation may be added according to appearance.
A structured elimination diet trial is appropriate when food allergy is possible. Biopsy is recommended when a lesion has an unusual appearance, fails to respond, is located in the mouth or could represent squamous cell carcinoma or another tumour.
There is no single screening blood test for eosinophilic granuloma complex. Early identification matters because repeated self-trauma and chronic inflammation make lesions larger and more difficult to control. Photographs can help document recurrence and response over time.
Diagnosis is often supported by cytology showing numerous eosinophils. Biopsy may be recommended when a lesion is atypical, persistent or could be confused with infection or cancer.
Anti-inflammatory treatment is used to settle active lesions, with corticosteroids being common for short-term control. Ciclosporin or other immune-modulating therapy may be considered for cats needing longer-term management. Secondary bacterial infection is treated when confirmed.
Trigger control is equally important. Reliable flea prevention is maintained for all household pets. A strict elimination diet is used when food allergy is suspected, and environmental allergy is managed when other causes have been excluded. Mosquito avoidance can be important in cats with seasonal lesions on exposed areas.
Painful oral lesions or severe plaques may need nutritional support, topical care or additional analgesia. Owners should avoid repeatedly applying over-the-counter creams because cats groom them off and some human ingredients are unsafe.
The prognosis is usually good when a trigger can be identified and controlled. Some cats require intermittent or long-term anti-inflammatory treatment. Recurrence after medication stops is a reason to revisit the underlying allergy plan rather than simply increasing treatment indefinitely. Any lesion that changes character or stops behaving like previous episodes should be reassessed and, if necessary, biopsied.
When long-term medication is required, periodic blood testing or other monitoring may be recommended according to the chosen drug and the cat’s age and health.
Treatment must address both the inflammation and the trigger.
