Dilated cardiomyopathy, or DCM, is a heart muscle disease in which the ventricles become enlarged and contract weakly. The heart therefore pumps blood forward less effectively, and pressure can build behind the failing chambers. DCM was once a relatively common feline disease when some commercial diets did not contain enough taurine. Modern complete feline diets are supplemented appropriately, so taurine-deficiency DCM is now uncommon, although the disease can still occur.
Affected cats may show lethargy, weakness, reduced appetite, rapid breathing or signs of congestive heart failure. Fluid can accumulate in or around the lungs, causing increased respiratory effort. Enlargement of the atria can also increase the risk of blood clot formation and arterial thromboembolism. Some cats collapse or develop abnormal heart rhythms.
Taurine is essential for normal feline heart and retinal function. Cats cannot produce enough for their needs and must obtain it from food. Cats fed nutritionally incomplete homemade diets, poorly formulated products or unusual diets are at greater risk of deficiency.
It shows enlarged chambers and reduced systolic contraction and helps distinguish DCM from other heart diseases. Taurine concentration may be measured when deficiency is possible.
The outlook depends heavily on cause and severity. Taurine-deficient cats can improve dramatically after supplementation, sometimes with substantial recovery of heart function. Cats with non-nutritional DCM or advanced heart failure have a more guarded prognosis and may require long-term cardiac treatment.
The nutritional history is therefore clinically meaningful. A cat eating a complete commercial feline food has a very different risk profile from one receiving a restricted homemade diet, dog food or a formulation not designed to meet feline taurine requirements. Diet history can directly influence both diagnosis and prognosis.
Early detection is difficult because DCM can remain clinically silent until pumping ability is substantially reduced.
Routine auscultation may reveal a murmur, gallop rhythm or abnormal heart rate, but a normal examination does not completely exclude disease.
Any cat with rapid sleeping respiratory rate, increased breathing effort, weakness, collapse or unexplained exercise intolerance should be assessed promptly. Cats fed a homemade, vegetarian, unbalanced or otherwise unusual diet deserve particular nutritional review because taurine intake may be inadequate.
Echocardiography is the key test when DCM is suspected. It measures chamber size and contraction and identifies fluid or atrial enlargement that affects prognosis. Chest radiographs help assess congestive heart failure, and electrocardiography can identify arrhythmias. Blood testing evaluates general health, and taurine concentration can be measured if dietary deficiency is plausible.
There is no general DNA screening test for feline DCM. Prevention relies primarily on feeding a nutritionally complete feline diet and investigating cardiac signs promptly. Cats recovering from taurine deficiency need repeat echocardiography because clinical improvement can precede full structural recovery, and supplementation should continue according to veterinary guidance.
However, not every case of DCM is caused by taurine, so a complete dietary history and cardiac assessment are necessary. Echocardiography is the main diagnostic test. Blood pressure, electrocardiography, chest imaging and blood tests may be added.
If taurine deficiency is suspected or confirmed, supplementation is started promptly while the diet is corrected to a complete and balanced feline formulation. Taurine is safe at therapeutic doses and may allow substantial recovery of heart muscle function, but improvement takes time and the cat may still need intensive heart-failure treatment initially.
Congestive heart failure is managed with diuretics, oxygen when needed and other cardiac medication selected from the individual findings. Cats with significant atrial enlargement may also receive antiplatelet treatment to reduce clot risk. Abnormal heart rhythms are treated when they affect circulation or cause clinical signs.
Nutrition should be reviewed carefully. Simply adding taurine to a fundamentally unbalanced homemade diet may not correct other deficiencies, so a complete dietary plan is important. Cats should not be switched to unconventional diets without veterinary nutritional guidance.
Follow-up echocardiography tracks whether contraction is improving. Kidney values, hydration and electrolytes are monitored when diuretics are used. Taurine-deficient DCM can have a favourable outcome when identified early, while non-responsive disease has a less predictable prognosis. Any recurrence of rapid breathing, weakness or collapse requires prompt reassessment.
