Diabetes mellitus occurs when a cat cannot regulate blood glucose effectively because insulin production is inadequate, the body becomes resistant to insulin, or both. Most feline diabetes resembles type 2 diabetes in people: insulin is still produced initially, but tissues respond poorly and pancreatic beta cells become increasingly stressed.
Typical signs are increased thirst, larger urine volumes, weight loss despite a good or increased appetite and reduced muscle condition. Some cats develop weakness in the hind legs and walk with the hocks dropped towards the floor because prolonged high glucose damages peripheral nerves. If diabetes becomes severely uncontrolled, diabetic ketoacidosis can cause vomiting, dehydration, profound lethargy, rapid breathing and collapse.
Obesity is an important risk factor because excess body fat contributes to insulin resistance. Increasing age, physical inactivity, pancreatitis and certain medications can also increase risk. Some breeds and individual cats appear more susceptible than others.
Stress can temporarily raise a cat’s blood glucose substantially, so a single high reading in the clinic does not always prove diabetes.
Feline diabetes is manageable, and some cats can enter remission when treatment begins early, body weight is controlled and glucose is brought under good control. Remission means insulin is no longer required at that time, not that the cat can never become diabetic again. Long-term monitoring remains important because relapse is possible.
Diabetes is not caused by sugar in the diet alone. It develops from a combination of insulin resistance and impaired insulin secretion, with obesity being one important modifiable contributor. This distinction matters because effective treatment addresses the whole metabolic problem rather than simply eliminating one ingredient from the cat’s food.
Early detection relies on recognising increased thirst, larger urine clumps, unexplained weight loss or an unusually strong appetite. Persistent hyperglycaemia is interpreted together with glucose in the urine and the cat’s clinical signs.
Routine senior-cat examinations that include weight trends, blood chemistry and urinalysis can identify glucose abnormalities before severe illness develops, particularly in overweight or otherwise high-risk cats.
A diagnosis should not be based on one stress-related blood glucose reading. Fructosamine can provide an estimate of average recent glucose and is useful when stress hyperglycaemia makes the diagnosis uncertain.
Once treatment begins, monitoring focuses on whether glucose control is safe and whether the cat may be entering remission. Home blood-glucose testing is often more representative than clinic-only measurements because it reduces stress effects. Continuous glucose monitors can also be useful in selected cats. Body weight, appetite, thirst, urine output and insulin dose are recorded alongside glucose results.
Any diabetic cat that becomes weak, vomits, stops eating or develops rapid breathing needs urgent assessment for ketoacidosis or another concurrent illness. Early identification of poor control allows insulin, diet or contributing disease to be adjusted before a crisis develops.
Diagnosis requires persistent high blood glucose together with glucose in the urine and compatible clinical signs. Fructosamine testing can help assess average glucose over the preceding weeks.
Most diabetic cats are treated with insulin injections given at consistent times each day. Modern feline-friendly insulin preparations can provide good control, and owners can usually learn injections quickly because the needles are small. The insulin dose is adjusted using clinical response and glucose monitoring rather than by changing it unpredictably at home.
Diet is an important part of treatment. Many cats benefit from a nutritionally complete, high-protein, lower-carbohydrate wet diet, although the best plan must account for kidney disease, gastrointestinal disease and other individual needs. Overweight cats need gradual weight loss because reducing insulin resistance can improve control and increase the chance of remission.
Home monitoring may include blood glucose, body weight, water intake and urine changes. Hypoglycaemia is an important risk of insulin therapy; sudden weakness, tremors, disorientation or seizures require emergency advice. If a cat does not eat normally, the insulin plan may need adjustment according to veterinary instructions.
Concurrent disease such as pancreatitis, dental disease or infection should be addressed because it can increase insulin requirements. Some cats enter remission after weeks or months of good control, but insulin should never be stopped without evidence that it is safe. Ongoing monitoring remains necessary even after remission because diabetes can return.
