Feline demodicosis is skin disease caused by overgrowth of Demodex mites. More than one Demodex species can affect cats, and the clinical pattern differs depending on the mite involved. Demodex cati lives in hair follicles and may be associated with underlying immune suppression, while Demodex gatoi lives more superficially and can be contagious between cats.
Affected cats can develop patchy hair loss, scale, crusting, redness and variable itch. Head and neck lesions are common with follicular disease, while D. gatoi often causes intense over-grooming and truncal hair loss. Because cats can groom away evidence of parasites, the skin may appear smoother than expected despite significant itch.
The distinction between species matters. A cat with D. cati may need investigation for conditions that reduce immune control, such as diabetes, corticosteroid exposure or other systemic disease. With D. gatoi, apparently healthy in-contact cats can carry mites and perpetuate infestation.
D. gatoi can be difficult to find because it is superficial and readily removed by grooming, so repeated sampling or a treatment trial may be needed in a strongly suspicious case.
Demodicosis is not automatically a sign of poor hygiene. The mite-host relationship and immune response determine whether disease develops. Most cases are treatable, but successful management may require both killing mites and addressing the underlying health problem or untreated contacts that allowed the infestation to persist.
The species distinction also changes the household implications. D. cati is generally linked to the individual cat and its health, whereas D. gatoi can spread between cats and may cause a cluster of itchy animals. Treating both patterns as the same disease can lead to missed underlying illness or repeated reinfection.
Deep skin scrapings and hair plucks can identify follicular D. cati mites.
Demodicosis should be considered in cats with unexplained patchy alopecia, scale, crusting or intense over-grooming after flea allergy and fungal disease have been assessed. The distribution of lesions and whether other cats are itchy can provide useful clues.
Superficial sampling, acetate tape or faecal examination may detect D. gatoi because cats ingest mites during grooming. A negative first sample does not completely exclude D. gatoi, especially in a very fastidious cat.
When D. cati is found, the veterinarian may recommend blood and urine testing or other investigation for immune-suppressing disease. Medication history is also reviewed because long-term corticosteroid use can affect mite control. In a multi-cat household with suspected D. gatoi, apparently normal contacts may need to be considered.
There is no routine screening programme for healthy cats. Early diagnosis prevents prolonged itch and unnecessary allergy treatment. Persistent self-trauma despite appropriate flea control is a reason to revisit mite testing rather than assuming all over-grooming is behavioural.
Diagnosis uses skin scrapings, hair examination, tape preparations or other sampling techniques.
Treatment uses a cat-safe antiparasitic medication effective against the Demodex species involved. Modern isoxazoline-class products are commonly used off label or according to regional approvals, and the veterinarian selects the product and schedule for the individual cat.
With contagious D. gatoi, all in-contact cats may need treatment even if some have minimal signs. Failure to treat carriers can lead to repeated reinfestation. Environmental treatment is less central than for fleas because the mites spend most of their life on the host.
Secondary bacterial infection or severe inflammation is treated when present. Cats with D. cati should also have any underlying immune-suppressing disease addressed; mite treatment may be less successful if diabetes, systemic illness or inappropriate corticosteroid exposure remains uncontrolled.
Clinical improvement can occur before every mite is eliminated, so the planned course should be completed. Rechecks and repeat sampling may be recommended in persistent cases. Prognosis is generally good for D. gatoi once household transmission is controlled. Recurrent D. cati warrants renewed investigation for an underlying health problem rather than simply repeating parasite treatment indefinitely.
If lesions return soon after an apparently successful course, confirming the mite species again and reviewing untreated contacts or systemic disease is more useful than simply increasing the dose.
