Chronic kidney disease, or CKD, is a progressive loss of kidney function that is especially common in older cats. Healthy kidneys remove waste products from the blood, regulate water and electrolytes, help control blood pressure and produce hormones involved in red blood cell production. As functioning kidney tissue is lost, these tasks become increasingly difficult.
Early CKD may cause few obvious signs because cats can compensate for substantial loss of kidney function. Increased thirst and larger volumes of urine are common early clues. With progression, cats may lose weight and muscle, eat less, vomit, become dehydrated, develop bad breath or appear less active. High blood pressure, low potassium, anaemia and excess phosphorus can develop and contribute to weakness or further organ damage.
CKD is not one single disease. Previous kidney injury, congenital abnormalities, chronic inflammation, infection and ageing changes can all lead to permanent loss of nephrons.
A cat that is dehydrated or acutely ill can have temporary kidney-value changes, so results must be interpreted in context.
Chronic kidney disease cannot usually be reversed, but progression and complications can often be managed for months or years.
One of the most useful ways to think about CKD is as a condition that changes gradually and needs repeated reassessment.
A trend over time can be more informative than a single result. Outward changes can include increased thirst, larger urine clumps, weight loss, reduced appetite, vomiting, poor coat quality and a gradual drop in activity. Because older cats may have more than one condition, these signs should not automatically be attributed to ageing. Blood pressure is important because hypertension can damage the kidneys, eyes, brain and heart. Early detection allows nutritional and blood-pressure management to begin before dehydration, severe weight loss or advanced complications make treatment more difficult.
CKD is often detected through routine senior-cat testing before severe signs appear. From middle age onward, regular health checks should include body weight, muscle condition, blood pressure, blood chemistry and urinalysis at intervals appropriate to the cat’s age and previous results.
Blood tests commonly include creatinine, urea, SDMA, phosphorus, potassium and red blood cell measurements. Urinalysis assesses concentration, protein and sediment. Urine culture may be appropriate in selected cats, as kidney infection can occur without dramatic urinary signs.
Ultrasound may be recommended when kidney shape, obstruction, stones, cysts or infection are concerns. Once CKD is diagnosed, monitoring frequency increases according to stage and stability.
In many older cats, no single original cause can be identified by the time disease is diagnosed. Diagnosis uses blood and urine testing together rather than relying on one value. Creatinine and SDMA help assess filtration, while urine concentration, urine protein, blood pressure and imaging provide additional information. Staging the disease helps determine which treatments are appropriate and allows monitoring to focus on hydration, nutrition, blood pressure, phosphorus, potassium, protein loss and quality of life. A treatment that is unnecessary in an early stable stage may become important later, while appetite and hydration sometimes deserve more immediate attention than trying to correct every laboratory abnormality at once.
Management is individualised according to disease stage and complications. A therapeutic renal diet is one of the most important long-term treatments for many cats. These diets control phosphorus, provide appropriate high-quality protein and are formulated to support cats with reduced kidney function. Diet changes should be gradual because maintaining calorie intake is also essential.
Hydration is supported with wet food, accessible water and, in some cats, subcutaneous fluids. Nausea, vomiting and poor appetite are treated so the cat continues eating. High blood pressure is managed with antihypertensive medication, significant urine protein loss may require additional treatment, and high phosphorus can be addressed with diet and phosphate binders when needed. Potassium supplementation and treatment of anaemia are appropriate in selected cases.
Regular monitoring guides adjustments because CKD changes over time. Weight, muscle condition, blood pressure, kidney values, phosphorus, potassium and urine findings are reassessed at intervals based on severity.
The aim is not to normalise every laboratory value but to maintain hydration, appetite, body weight and comfort while slowing avoidable progression. Sudden deterioration should be investigated rather than assumed to be inevitable CKD progression, because dehydration, infection, obstruction or another illness may be treatable. Many cats maintain a good quality of life for extended periods with consistent monitoring and supportive care.
