Feline chronic gingivostomatitis is a severe, persistent inflammatory disease of the mouth. Unlike ordinary gingivitis limited to the gumline, inflammation often extends across the tissues at the back of the mouth, around the teeth and sometimes beneath the tongue. The condition is extremely painful and can interfere with eating, grooming and normal social behaviour.
The exact cause is complex. The disease appears to involve an exaggerated immune response to plaque and other oral antigens, and viruses such as feline calicivirus are associated with many cases. However, detecting a virus does not by itself explain or predict the severity of disease. Individual immune response, dental disease and oral bacteria all contribute.
Affected cats may drool, have very bad breath, cry or jump away when trying to eat, paw at the mouth, lose weight or stop grooming. Some approach food because they are hungry and then retreat because chewing is painful. Blood-tinged saliva and extensive red, ulcerated tissue may be visible. Chronic pain can also make a cat withdrawn, irritable or reluctant to be touched around the head.
This is usually a long-term disease rather than a simple infection. Antibiotics may temporarily reduce secondary bacterial activity but do not correct the underlying immune response. Successful management requires aggressive dental control and, in some cats, additional medication to reduce ongoing inflammation.
The disease can be emotionally difficult for owners because it may require removal of apparently normal-looking teeth. The reason is functional rather than cosmetic: reducing plaque-retaining surfaces can quiet the abnormal immune response and remove sources of dental pain that are otherwise difficult to control.
Outward changes can include drooling, foul breath, weight loss, reduced grooming, pawing at the mouth, food being dropped or a cat repeatedly approaching food and then backing away are important warning signs. Prolonged severe oral inflammation produces continuing pain, weight loss and progressively greater difficulty eating and grooming.
Persistent oral pain or widespread inflammation should be investigated early. Routine veterinary examinations can identify gingivitis before owners notice difficulty eating, but chronic gingivostomatitis is suspected when inflammation extends beyond the immediate gum margin, particularly into the tissues at the back of the mouth.
A cat that stops eating because of oral pain needs prompt veterinary care.
Assessment normally includes a complete oral examination under general anaesthesia and full-mouth dental radiographs. These are used to identify periodontal disease, tooth resorption, retained roots and other painful dental problems. Blood tests may be used to assess general health and to guide anaesthesia. Testing for feline immunodeficiency virus, feline leukaemia virus or calicivirus may be considered depending on the individual case, but no single positive test confirms the cause of gingivostomatitis.
Biopsy is appropriate when lesions are asymmetric, mass-like or otherwise atypical.
Diagnosis is based on the characteristic oral inflammation after other causes of severe mouth disease have been considered. A full dental assessment under general anaesthesia with dental radiographs is essential because retained roots, tooth resorption and periodontal disease can perpetuate pain. Biopsy may be recommended when the appearance is unusual or cancer cannot be confidently excluded.
The most effective treatment for many cats is extraction of teeth that provide surfaces for plaque accumulation, often including premolars and molars and sometimes all remaining teeth. This sounds drastic, but cats generally adapt extremely well, and removal of the major plaque-retaining surfaces can produce substantial or complete improvement. Full-mouth dental radiographs are important to ensure diseased roots are not left behind.
Pain control and nutritional support are central during treatment. Some cats need soft food or temporary feeding support while the mouth heals. Antibiotics may be used for specific secondary infections, but repeated antibiotics alone do not provide lasting control.
Not every cat responds completely to extraction. Persistent cases may need anti-inflammatory or immune-modulating medication selected by the veterinarian, with regular reassessment for side effects and response. Good plaque control remains useful where teeth are retained, although brushing may be impossible while the mouth is painful.
The prognosis is variable. Many cats improve greatly after appropriate dental extraction, while others require long-term medical management. The practical goal is comfortable eating, stable weight, normal grooming and the lowest level of medication needed to control inflammation. Ongoing oral examinations remain important even after extensive extraction because inflammation can persist in toothless areas.
