Chronic bronchitis is persistent inflammation of the lower airways that causes coughing and increased mucus production. It overlaps clinically with feline asthma, but chronic bronchitis is generally characterised by neutrophilic airway inflammation rather than the eosinophilic pattern and reversible bronchospasm typical of asthma. Some cats show features of both disorders.
The main sign is a recurrent cough lasting weeks or months. Cats may crouch low and extend the neck during a coughing episode, which can be mistaken for retching. Wheezing, increased respiratory effort or reduced activity can occur, but severe acute bronchospasm is less characteristic than in asthma.
There is no single cause in many cats. Long-term exposure to smoke, dust, aerosols or other inhaled irritants can worsen airway inflammation. Infection, lung parasites and structural disease need to be considered because they can mimic chronic bronchitis. Heart disease can also produce respiratory signs, although coughing is less typical of feline cardiac disease than many owners expect.
The cat must be stable enough for anaesthesia before airway sampling is attempted.
Chronic bronchitis is usually managed rather than cured. Ongoing inflammation can permanently remodel the airways if poorly controlled.
The distinction from asthma matters because an individual cat’s inflammatory pattern influences treatment emphasis, yet the two conditions can overlap.
Persistent rapid breathing or increased effort between coughing episodes is more urgent. They may show a bronchial pattern but cannot reliably distinguish asthma, chronic bronchitis, infection or parasites on their own. Airway sampling provides more specific information when the cat is stable enough. Chronic cough can precede severe airway remodelling and loss of comfortable respiratory reserve. The goal is not necessarily to eliminate every occasional cough but to prevent persistent inflammation, worsening respiratory effort and progressive airway damage.
A cough that persists or recurs for more than a brief period should be investigated. Owners can record episodes on video and note frequency, relation to exercise, litter changes, smoke exposure and other possible triggers.
Chest radiographs are commonly the first imaging test. Blood testing and faecal or parasite tests may be added according to risk.
Cytology can show whether neutrophils, eosinophils or mixed inflammation predominate, and bacterial culture can identify infection that requires targeted antibiotics. Cardiac assessment is considered when imaging or examination suggests heart disease.
There is no routine screening test for cats without respiratory signs. Open-mouth breathing, cyanosis, collapse or marked respiratory effort requires emergency assessment rather than routine bronchitis testing.
Diagnosis uses history, chest imaging and, when appropriate, airway sampling. Bronchoalveolar lavage allows cytology and culture to distinguish inflammatory patterns and identify infection. Parasite testing is selected according to geographic and lifestyle risk. A cat with long-term cough may therefore need repeated reassessment rather than a permanent label based on one episode or one radiograph.
Treatment usually includes anti-inflammatory medication, most commonly corticosteroids. Oral medication may be used initially, while inhaled corticosteroids can be helpful for long-term control in cats trained to accept a spacer and mask. Bronchodilators may be added when airway narrowing or wheeze is present.
Antibiotics are not used automatically. They are most appropriate when bacterial infection is documented or strongly suspected from airway culture and cytology. Lung parasites are treated specifically when identified. Environmental irritants such as tobacco smoke, vaping aerosols, strong fragrances and dusty litter should be reduced because they can perpetuate airway inflammation.
Hydration and maintaining normal body weight support respiratory function. Some cats benefit from humidification or other measures that help mobilise thick airway secretions, but these should be discussed with the veterinarian rather than improvised during respiratory distress.
Chronic bronchitis often requires long-term treatment. Medication is adjusted according to cough frequency, breathing comfort and repeat assessment. Sudden deterioration should prompt reassessment for asthma-like bronchospasm, infection or another new problem.
Treatment therefore aims to reduce inflammation, improve mucus clearance, minimise irritants and preserve comfortable breathing over the long term.
