Cholangitis is inflammation of the bile ducts, while cholangiohepatitis describes inflammation that also involves surrounding liver tissue. In cats, the major inflammatory patterns include neutrophilic cholangitis, which can involve bacterial infection, and lymphocytic cholangitis, which has a more chronic immune-mediated pattern.
Neutrophilic disease can occur when bacteria ascend from the intestine through the bile duct. Cats may develop fever, vomiting, poor appetite, abdominal pain and jaundice. Lymphocytic disease often progresses more slowly and can produce weight loss, intermittent appetite change, fluid accumulation or chronic liver abnormalities.
Feline anatomy links the bile duct closely with the pancreatic and intestinal ducts, which helps explain why cholangitis can occur alongside pancreatitis and inflammatory bowel disease. The combination is often described as triaditis.
The distinction matters because treatment differs. Antibiotics are central for bacterial neutrophilic cholangitis, while chronic lymphocytic disease may require immunomodulation once infection has been excluded. Nutritional support is important in both because cats that stop eating are at risk of hepatic lipidosis.
Prognosis ranges from good in appropriately treated acute bacterial disease to more variable in chronic inflammatory forms. Repeated “liver enzyme” abnormalities should be investigated for cause rather than treated as a diagnosis by themselves.
The gallbladder and bile ducts should be considered part of the diagnostic picture rather than focusing only on liver enzymes. Bile-flow obstruction, bacterial infection and immune-mediated inflammation can create overlapping biochemical changes but require very different treatment decisions. Sampling can therefore be more informative than repeatedly adjusting non-specific liver supplements.
A dilated duct or abnormal gallbladder can increase suspicion for obstruction or infection but does not determine the inflammatory type alone. Cats with known pancreatitis or intestinal disease may need liver evaluation when new biochemical or clinical signs develop.
Jaundice, persistent appetite loss, vomiting, fever or unexplained liver-value elevations should prompt further investigation. The veterinarian assesses whether the pattern suggests hepatocellular injury, cholestasis or obstruction and considers concurrent pancreatic and intestinal disease.
Abdominal ultrasound evaluates liver texture, gallbladder, bile-duct diameter, pancreas and intestines.
Bile collected by ultrasound-guided cholecystocentesis can be cultured when bacterial cholangitis is suspected. Liver cytology or biopsy provides histological classification and may be necessary for chronic or ambiguous cases. Coagulation status is checked before invasive sampling.
There is no routine screening test in healthy cats. Early detection depends on investigating persistent liver abnormalities and appetite change rather than waiting for severe jaundice.
Blood clotting tests are important before liver biopsy because significant hepatobiliary disease can alter coagulation and increase procedural risk.
Diagnosis uses blood chemistry, bilirubin and ultrasound, but imaging alone cannot always identify the inflammatory type. Sampling of bile for culture is particularly valuable when bacterial infection is suspected. Liver aspirates or biopsy may be needed to distinguish neutrophilic, lymphocytic, neoplastic and other liver disease.
Neutrophilic cholangitis is treated with antibiotics that achieve good biliary penetration, ideally selected from bile or liver culture. Treatment may need to continue for several weeks. Fluids, pain relief, anti-nausea medication and nutritional support are provided according to severity.
Lymphocytic cholangitis may require corticosteroids or other immune-modulating treatment once active bacterial infection has been considered and excluded. Ursodeoxycholic acid can support bile flow in selected cats when there is no complete obstruction. Antioxidant or liver-support medications may be added according to the individual case.
Cats that are not eating need active nutritional support to prevent or treat hepatic lipidosis. Concurrent pancreatitis and intestinal disease are managed at the same time rather than assuming liver treatment alone will resolve all signs.
Follow-up blood tests monitor bilirubin and liver values, while repeat ultrasound or culture is used when response is incomplete. Recurrence can occur, particularly in chronic disease. Prognosis is often favourable for acute neutrophilic cholangitis treated promptly, while lymphocytic disease may require long-term management.
Cats with chronic lymphocytic disease may need treatment tapered slowly over months, with relapse recognised by appetite, weight and biochemical changes rather than waiting for obvious jaundice.
