Chlamydia felis is a bacterial organism that infects the conjunctiva, the delicate membrane lining the eyelids and covering part of the eye. It is a recognised cause of feline conjunctivitis, particularly in young cats living in close-contact groups. Unlike many feline respiratory pathogens, its most characteristic feature is persistent eye inflammation rather than severe disease of the lungs.
Infection often begins in one eye and spreads to both. The conjunctiva becomes red and swollen, with watery discharge that may become thicker. The eyelids can look puffy and uncomfortable. Mild sneezing or nasal discharge can occur, but mouth ulcers and severe lower respiratory signs are not typical features of uncomplicated Chlamydia felis infection.
Transmission requires relatively close contact because the organism survives poorly outside the host. Multi-cat households, breeding colonies and shelters therefore provide the most favourable conditions for spread. Kittens are particularly susceptible once maternally derived protection declines.
The infection is treatable with appropriate antibiotics. The key practical issue is using a drug that reaches the intracellular organism and treating for long enough to reduce relapse. When several cats in one household are affected, management may need to address exposed contacts as well as the cat with the most obvious eye signs.
The organism has a particular preference for conjunctival cells, which is why eye disease dominates the clinical picture. When a cat labelled with “chlamydia” has severe coughing, pneumonia or extensive mouth ulceration, the veterinarian should look for concurrent pathogens or an alternative diagnosis rather than forcing every sign into one explanation.
Red, swollen conjunctiva with discharge that begins in one eye and then involves both is a common pattern. Significant corneal ulceration or severe respiratory distress suggests that another or additional disease may be present.
Persistent conjunctivitis in a kitten or young cat, especially in a multi-cat environment, should raise suspicion for Chlamydia felis.
A veterinarian examines the eyes and may perform fluorescein staining to rule out corneal ulceration. Conjunctival swabs for PCR are useful when confirmation will affect treatment, when multiple cats are involved or when disease has not responded as expected.
Testing every healthy cat is not generally necessary. In breeding colonies or shelters with repeated conjunctivitis, targeted testing and review of vaccination and group-management practices can help control outbreaks. Recently treated cats can occasionally remain PCR positive for a period, so laboratory findings are interpreted with clinical response.
Early detection matters mainly because prompt treatment reduces discomfort and transmission. Eye pain, marked swelling, inability to open the eye or a cloudy cornea requires more urgent assessment because those findings are not adequately explained by simple conjunctivitis alone.
Diagnosis can be based on the clinical pattern, but PCR testing from conjunctival swabs can confirm infection in outbreaks or persistent cases. Results should still be interpreted alongside the examination because more than one respiratory or ocular pathogen can be present at the same time.
Chlamydia felis is treated with an antibiotic that reaches the organism inside cells. Doxycycline is commonly used and treatment generally continues for a sufficient period beyond resolution of obvious signs to reduce relapse. The exact course should be prescribed by the veterinarian.
Eye ointment alone may not be enough because infection can involve both eyes and other mucosal sites even when one eye looks worse. Systemic treatment is therefore usually preferred. Eye discharge can be cleaned gently, and painful or severely inflamed eyes are reassessed to make sure corneal disease is not being missed.
When several cats share a household, exposed cats may also need assessment or treatment because close contact allows transmission. Bedding, bowls and hands should be kept clean, although environmental survival is shorter than with some viral respiratory pathogens.
Vaccination against Chlamydia felis is available in some settings and may be considered for cats at increased group-housing risk, but it is not a core vaccine for every pet cat. The prognosis is usually very good with appropriate therapy. Persistent or recurrent conjunctivitis after a full course should prompt reassessment for treatment adherence, reinfection or another ocular pathogen rather than repeated empirical medication indefinitely.
