A cataract is opacity within the lens of the eye. The normal lens is transparent so light can pass through and focus on the retina. When lens fibres lose their transparency, vision becomes reduced in proportion to the size, density and location of the opacity.
Cataracts are less common in cats than in dogs. They may develop after severe eye inflammation, trauma, lens luxation, inherited disease or developmental abnormalities. Diabetes causes cataracts commonly in dogs but much less often in cats because feline lenses handle glucose differently.
A small cataract can be an incidental finding with little effect on vision. A mature cataract may make the pupil appear white or grey and can substantially reduce sight. Owners may notice hesitation in unfamiliar areas, difficulty judging jumps or increased startle responses. Cats often adapt well at home because they rely heavily on smell, hearing and memory.
It is important to distinguish a cataract from normal age-related nuclear sclerosis, corneal cloudiness and inflammatory material in the eye. Retinal function must be assessed before cataract surgery is considered.
Cataracts do not always need removal. Inflammation caused by the cataract can become a problem even when the cat already adapts to reduced sight, so ongoing ophthalmic monitoring remains important.
The cause also influences how the cataract behaves. A developmental opacity in a young cat may remain stable for years, whereas a cataract associated with chronic uveitis can continue to progress alongside painful inflammation. Describing every lens opacity simply as “old age” risks missing a treatable underlying eye disease.
The pupils may be dilated so the entire lens and retina can be assessed. Outward changes can include hesitation in low light, bumping into unfamiliar objects or misjudging jumps can indicate declining vision.
Routine eye examinations can identify early lens changes before the owner notices visual problems. Cats with previous uveitis, trauma or inherited ocular risk may need more frequent assessment. Any new white or grey appearance behind the pupil should be examined rather than assumed to be normal ageing.
The veterinarian uses a slit lamp or other magnification to determine whether the opacity is truly within the lens. Eye pressure and inflammation are checked because glaucoma or uveitis can alter treatment decisions.
When the cataract is too dense to see the retina, ocular ultrasound can assess the back of the eye. Electroretinography may be recommended before surgery to confirm that the retina can function. Breed-specific genetic testing is relevant only for recognised inherited cataracts and does not replace an eye examination.
Sudden redness, pain or rapid cloudiness is more urgent because cataracts associated with inflammation or lens instability may need prompt treatment.
A veterinary eye examination localises the opacity to the lens. Dilated examination and, when the back of the eye cannot be seen, ocular ultrasound may be required.
A cataract that is small, stable and not causing inflammation may simply be monitored. When the opacity causes significant visual impairment and the rest of the eye is healthy, cataract surgery by phacoemulsification can restore useful vision. The cloudy lens is broken up and removed, and an artificial lens may be implanted when appropriate.
Cats require detailed preoperative assessment because retinal disease, uncontrolled uveitis or glaucoma can make surgery unsuitable. Postoperative care is intensive and includes multiple eye medications, activity restriction and frequent rechecks while inflammation settles.
Cataracts caused by uveitis or trauma may need treatment of the underlying ocular disease even if surgery is not planned. Anti-inflammatory medication is used when appropriate, but the exact drug depends on corneal integrity and other findings. A blind but comfortable eye does not necessarily require surgery.
The prognosis for surgery is best when the eye is otherwise healthy and inflammation is controlled. Untreated mature cataracts can occasionally contribute to chronic lens-induced uveitis, so long-term monitoring remains important. Cats that cannot undergo surgery often adapt well if the home environment remains predictable and safe.
Treatment depends on visual impairment, progression and the health of the rest of the eye.
