Arterial thromboembolism occurs when a blood clot forms, usually in an enlarged left atrium, breaks free and travels through the arterial circulation until it lodges in a smaller vessel. In cats, the classic site is the aortic trifurcation supplying the hind legs, producing what is commonly called a saddle thrombus. Cardiomyopathy is the most common underlying problem.
The onset is usually sudden and dramatic. Affected cats may cry out in severe pain, become unable to use one or both hind legs and develop cold paws with weak or absent pulses. The paw pads may appear pale or bluish because blood flow is reduced. Clots can also lodge in vessels supplying a front limb, kidney, brain or other organs.
The emergency is not only a limb problem. Many cats also have underlying congestive heart failure, and the stress of the event can worsen breathing. Damaged muscle releases potassium and other substances as circulation changes, which can create additional metabolic complications. The extent of tissue injury depends on how complete and prolonged the loss of blood flow has been.
Prognosis is guarded and varies with the number of limbs affected, body temperature, severity of heart disease and whether useful limb function begins to return. Some cats recover meaningful mobility over days to weeks, while others have extensive tissue injury or recurrent thromboembolism.
The pain is caused by sudden loss of oxygen to the affected muscles and nerves, not by a spinal injury. That difference is important because the limbs can appear paralysed even though the primary disease is cardiovascular. The event should always trigger investigation of the heart once the cat is stable.
The cardiologist evaluates atrial size, blood-flow velocity and evidence of clot formation. A cat that has already experienced one arterial thromboembolism is at increased risk of recurrence, so follow-up focuses on both limb recovery and ongoing control of the underlying heart disease.
There is no reliable screening test that predicts exactly when a clot will occur. Prevention therefore focuses on identifying cats with heart disease that have features associated with increased thromboembolic risk, particularly marked left atrial enlargement or spontaneous echocardiographic contrast.
Cats with known cardiomyopathy should have echocardiographic reassessment at intervals based on disease severity. These findings help determine whether preventive antiplatelet treatment is appropriate.
At home, sudden painful hindlimb weakness, dragging of the legs, cold paws, collapse or acute distress is an emergency. The cat may also breathe rapidly or with an open mouth if heart failure is present. This is not a condition to monitor overnight.
Auscultation alone cannot determine clot risk because some cats with substantial cardiomyopathy have no murmur. Echocardiography provides the most useful cardiac risk assessment.
Diagnosis is usually based on the characteristic clinical findings together with assessment for underlying heart disease. Echocardiography is important once the cat is stable enough. Blood tests, blood pressure and chest imaging may be required to evaluate systemic effects and heart failure.
Emergency treatment prioritises strong pain relief, stabilisation of breathing and management of underlying heart failure. Oxygen and minimal handling may be necessary if the cat is in respiratory distress. Antithrombotic medication is used to reduce further clot formation, but aggressive attempts to dissolve a clot are not routinely appropriate for every case because of bleeding and reperfusion risks.
The limbs are monitored for return of warmth, movement and sensation. Physiotherapy may be introduced carefully as circulation and comfort improve. Tissue damage can occasionally lead to skin or muscle complications that require additional treatment. Kidney values, potassium and hydration are monitored because systemic effects can evolve after the event.
Long-term management targets the heart disease and recurrent clot risk. Clopidogrel is commonly used for secondary prevention, with additional cardiac medication based on echocardiographic findings and whether congestive heart failure is present.
Recovery is variable. Some cats regain useful walking ability within days to weeks, while others do not. Quality of life, pain control, extent of paralysis and severity of heart disease all influence decisions. Cats that recover need close follow-up because recurrence is common. Any new limb weakness or breathing difficulty requires immediate reassessment.
