Acute kidney injury, or AKI, is a sudden reduction in kidney function occurring over hours to days. Unlike chronic kidney disease, where damage develops gradually, AKI can make a previously well cat seriously ill in a short period. Causes include toxins, severe dehydration or shock, infection, urinary obstruction, reduced blood flow to the kidneys and some medications or systemic diseases.
Cats may become lethargic, stop eating, vomit, become dehydrated or show changes in urine production. Some produce very little or no urine, while others continue to pass urine despite severe kidney dysfunction. Oral ulcers, bad breath, weakness and neurological signs can occur as waste products accumulate. The severity of outward signs does not always match the amount of kidney damage.
Important feline toxins include lilies, which can cause catastrophic kidney injury after even small exposures. Ethylene glycol and certain medications are other potential causes. Rapid treatment after a known exposure is crucial because some toxins cause irreversible damage once absorbed.
The veterinarian looks for evidence of dehydration, obstruction, infection or toxin exposure and considers whether pre-existing chronic kidney disease may also be present. Urine output is monitored closely because it affects treatment and prognosis.
AKI can sometimes be reversible if the cause is identified and treatment begins early. In other cases permanent kidney damage remains, leaving the cat with chronic kidney disease after recovery. The condition requires intensive veterinary care because fluid balance, electrolytes and waste products can change rapidly.
Unlike chronic disease, the kidneys in AKI may recover meaningful function if enough viable tissue remains. The first days are therefore critical. Identifying a reversible cause, preventing further exposure and supporting circulation while the kidneys recover can make the difference between temporary illness, permanent CKD and a fatal outcome.
Early detection depends on recognising risk and acting quickly when exposure or illness occurs.
There is no routine screening test that predicts an unexpected acute kidney injury in a healthy cat. Any cat that may have chewed or contacted a lily should receive immediate veterinary advice even if it appears completely normal.
Reduced appetite, repeated vomiting, sudden lethargy, dehydration, collapse or a marked change in urine production also justify urgent assessment. A cat producing little or no urine is particularly concerning. Owners should provide the veterinarian with information about plants, medications, chemicals, recent anaesthesia, illness and possible urinary obstruction.
Blood testing measures creatinine, urea, electrolytes, phosphorus and other values, while urinalysis assesses urine concentration, sediment and possible toxin-related changes. Blood pressure and urine output are monitored. Ultrasound or radiographs may be used to investigate obstruction, kidney size, stones or other structural causes. Urine culture is appropriate when infection is suspected.
Cats receiving medications that can affect kidney function or those undergoing major illness may need planned monitoring. In AKI, trends over hours and days are often more informative than a single result. Rapid recognition offers the best chance to remove the cause before permanent injury progresses.
Diagnosis relies on history, examination, blood tests and urinalysis, often combined with imaging.
Treatment is directed at the cause while supporting kidney function and maintaining safe fluid and electrolyte balance. Intravenous fluids are commonly used, but the amount must be matched carefully to hydration and urine output. Giving excessive fluid to a cat that cannot produce urine can be dangerous.
Specific therapy depends on the cause. Recent toxin exposure may require decontamination and intensive fluid therapy. Bacterial kidney infection is treated with appropriate antibiotics. Urinary obstruction must be relieved. Nausea, vomiting, pain and poor appetite are treated so the cat can maintain nutrition during recovery.
Electrolytes and acid-base balance are checked repeatedly. Cats producing very little urine may need specialist management, and severe cases can be considered for dialysis where available. Nutritional support becomes important when appetite does not return promptly.
Recovery varies widely. Kidney values may improve over days, but full recovery can take weeks, and some cats are left with chronic kidney disease. Follow-up blood pressure, blood tests and urinalysis are therefore important after discharge even if the cat appears normal. Prevention includes keeping lilies, antifreeze and unsafe medications inaccessible and never giving a cat human medication unless specifically directed by a veterinarian.
